End of Life Care is support provided to those who are in the last few months or years of their life, and wish to spend their remaining time in the comfort of their own home. We strive to make sure that the client lives as well as possible, and when the time comes, they die with dignity.When assembling your care package, we will take into account your wishes and preferences, which will be fully explored on your initial assessment.

 

We also recognise that it is also a difficult time for your friends and family, and so will aim to make life a little bit easier for them where possible. We will also work closely with other health professionals involved in your care, including GPs, Occupational Therapists, Physiotherapists, and Nurses.
